To treat you we collect your name, contact details, date of birth, medical and dental history, insurance details and the clinical records created during your care, including images and X-rays.
To provide dental treatment, to bill for it, and to run the practice. We may share the minimum necessary with your insurer to settle a claim, with a specialist if we refer you, or with a laboratory making your restoration.
We do not sell your information. We do not share it for marketing. We do not give it to anyone else without your authorisation except where the law requires it.
The forms on this site collect only what you type into them, and are used to respond to your enquiry. Please do not send detailed medical information through a web form — call the office instead.
